Astana-Premier Tech's Ion Izagirre has taken the victory of the fourth stage of Itzulia Basque Country while Brandon McNulty took the GC lead.

The route of the day took the peloton between Vitoria-Gasteiz and Hondarribia over a 189.2-kilometre course that included two climbs of the third category, one of the second and a last one categorised the first before descending to the finish line. The escape of the day was formed later on the stage, after the 100-kilometre mark, and included riders like Guillaume Martin, Ben O'Connor, Jefferson Cepeda and Juan Pedro Lopez.

Before the start of the last climb of the day, the pack caught the remaining escapees and almost right away several attacks happened, including some of McNulty and Mikel Landa (Bahrain Victorious), which were repelled by the rest of the reduced pack. But soon after a group of six -McNulty, Esteban Chaves, Izagirre, Pello Bilbaio, Jonas Vingegaard and Emmanuel Buchman- created a gap with the rest that was successful until the end.

The group sprinted for victory and the Spaniard of Astana-Premier Tech took the victory on homeland. The pack arrived 49 seconds after and this put McNulty on top of the overall standing, 23 seconds ahead of Primoz Roglic (Jumbo-Visma), 28 seconds of Vingegaard, 36 seconds from Bilbao and 43 from Pogacar.
